Thank you for your interest in participating in our March 28th Passport Day at Cleveland Public Library- Rockport Campus. Please complete the form below to request an appointment time. Appointments will be scheduled based on availability. You must select your top three (3) preferred time slots. You will receive a confirmation email with your final scheduled time within 5 business days or no later than 2 days before the event.
You will need to bring the following to your appointment:
ADULTS- Completed DS-11 application found on travel.state.gov | Proof of citizenship. MUST BE ORIGINAL | Photo ID | Photo copy of Proof of Citizenship and FRONT and BACK of Photo ID | COSTS: Passport Book-$130; Passport Card- $30; Passport Book & Card- $160. CHECK OR MONEY ORDER made out to "U.S. Department of State" | Facility Acceptance Fee- $35 can be paid by CHECK, MONEY ORDER, OR CARD
**BOTH PARENTS MUST BE PRESENT FOR CHILD PASSPORT APPOINTMENTS. If one cannot be, you MUST provide completed supporting documents found on travel.state.gov **
CHILDREN- Completed DS-11 application found on travel.state.gov | Proof of citizenship. MUST BE ORIGINAL. | Photo ID of BOTH parents | Photo copy of Proof of Citizenship and FRONT and BACK of Photo IDs | COSTS: Passport Book-$100; Passport Card- $15; Passport Book & Card- $115. CHECK OR MONEY ORDER made out to "U.S. Department of State" | Facility Acceptance Fee- $35 can be paid by CHECK, MONEY ORDER, OR CARD
FREE Passport Photos will be taken courtesy of Cleveland Public Library.
NO Passport Renewals will be processed. They must be done by mail or online.
