Veterans Recognition Ceremony
We are honored to invite you to participate in the Veterans Recognition Ceremony. This event will honor the service and sacrifice of veterans throughout Ohio’s 11th Congressional District and provide an opportunity for our community to recognize those who have served our nation.
We also welcome nominations for community members, family members, and organizations who have demonstrated an outstanding commitment to supporting veterans and military families. Their dedication deserves recognition alongside those who have served.
Please complete the form below to nominate yourself, a veteran, or community partner for recognition during the ceremony.
Eligibility Criteria
- Veteran must be a former member of the United States Armed Forces.
- Veterans from all branches, eras, and components (Active Duty, National Guard, and Reserve) are eligible.
- Veteran must have a DD214.

All nominations must be submitted by September 30. Additional event details, including the ceremony date, time, and location, will be shared with participants closer to the event.
